The economics of running an NHS dental practice are under more pressure than at any point in recent memory.

Rising overhead costs, National Minimum Wage increases, the cost of retaining skilled staff - the numbers are moving in a direction that makes long-term viability harder to sustain within an NHS contract model.

Today, Patient Plan Direct has published the State of NHS Conversion 2026, and it provides something that I think is genuinely valuable for any practice owner doing the financial analysis: a clear picture of what conversion actually costs and what it returns, built from real outcomes rather than projections.

The data that stands: on average, just 40% of an active patient base needs to convert onto a plan for the model to be commercially viable. For many practices, that threshold is lower than the numbers they are currently chasing to stay afloat within the NHS.
