For that you will need one color photograph, Pan Card, Address Proof (Passport, Ration Card, Registered Lease / Sale Agreement Of Residence, Driving License, Latest Bank Account Statement, Latest Landline Telephone Bill, Latest Electricity Bill, Latest Gas Bill, Aadhar Card), Cancelled Cheque of bank through all transitions you want to make.

7. Sign the form (Nominee: Page No-2, Investor: Page No: 3,6, Bank mandate form (Page No: 7) (Signature as per Bank), paste color photograph on KYC form (Page No: 8) and sign across it and one more signature on same form) (Total Signature: Nominee: 1, Investor: 5)
