The standard employer policy covers one to two times your annual salary. For a physician earning $300,000, that's $300,000 to $600,000 in coverage. That number sounds substantial until you measure it against what it would actually need to replace.

For a physician with a young family, $600,000 in coverage can disappear quickly. What remains isn't a safety net, but a bridge that doesn't quite reach.

Dr. Max Epstein could have chased the biggest paycheck. He didn't.

He was on the orthopedic surgery track, the high-volume, high-prestige, highest-paid route, when he asked a question most physicians don't ask until it's too late: "I know I could be happy doing this in 5 years. But will I be happy in 30?"

He picked the 30-year answer. He became a pain medicine physician. He moved to Utah. He built a four-day work week. And he started saving in a Roth as a resident.

Residency trains you to put your life on hold, including your finances. You defer your student loans, your savings, and your strategy because youโ€™re focused on the finish line.

But when the attending paycheck hits, that "deferral habit" doesn't just switch off. The income is finally there, but the big decisions get pushed to the back of the line again.
