A reminder for practice owners as we head into the fourth quarter.

You can spend all fall planning, setting goals, running numbers, mapping out next year. But a plan you don't act on is just a wish with better formatting. And a lot of the moves that matter have a hard deadline: December 31.

Some of them are money moves with a clock - equipment that has to be in service by year-end, owner payroll that has to run inside the calendar year, your final estimated tax payment. The other one is quieter but just as important: finishing next year's plan now, so you start January already in motion instead of still deciding.

Both land on the same date. This week's post walks through what to do about each, before the holidays swallow the time.

Here's a planning mistake that caps a lot of good practices.

When owners set next year's plan, most build it to run at 100%, every slot full, every day maxed. It feels productive. But it's fragile (one bad week and you miss) and it leaves you nowhere to grow, because if you're already maxed just to hit the goal, you can never beat it.

The fix is to model 80% as full capacity. Build the plan so hitting your number takes about 80% of what you and your team could do. That last 20% becomes your cushion for the messy weeks and your runway to exceed the goal. Just make sure the 80% works for your staff's schedule too, not only yours.

This week's post breaks down the whole idea, and what to do once you know whether your practice can carry the goal.

A quick exercise for any practice owner setting next year's number.

It's easy to say "I want to make $1.2 million." It's harder, and far more useful, to know what that means day to day. So break it down.

$1.2 million a year, across about 184 clinical days, is roughly $6,525 a day. At an average of $500 per patient, that's about 14 patients a day, four days a week. Suddenly the goal isn't a vague number. It's a specific, visible target you can look at and ask: is that doable on my current schedule, or does something have to change?

That's the whole idea behind what I call Business Model Math, and it's what turns a goal into an actual plan. This week's post walks through it step by step.

A better way to set next year's goal, for any practice owner staring at a blank plan right now.

Most of us set the number backward. We take last year, add ten percent, and call it a target. But that number isn't tied to anything, which is exactly why it's so easy to abandon the first hard month.

Try it the other way. Start with the life you want - your household, your kids' college, the trips, enough to retire when you'd like to - and price it out. That total is your real target, and now the practice has a job with your actual life underneath it. Same size number, maybe. Completely different pull.

The best part: working backward tells you something a ten percent bump never can. Whether you're on track, already past enough, or short with time to fix it.
This week's post walks through the whole thing.

A reframe to end the month on, for any practice owner who feels like they work hard and never quite get ahead.

All month I've been writing about what you owe - the IRS, your break-even, your estimated taxes. But there's one debt nobody ever sends you a bill for: the one you owe yourself.

Think about it. You never miss payroll. You never miss a tax deadline. Everyone else gets paid on time. But your own paycheck, your own savings, and the goals you started this practice for tend to get pushed to "next year," over and over. You've basically become a creditor in your own business, and you keep letting it pay you back last.
This week's post is about what you owe yourself and how to move yourself up the list.

Because the practice was always supposed to fund your life, not the other way around.

Quick one for practice owners who just made a quarterly tax payment.

You sent the IRS a check on the 15th. Do you know if it was the right amount? Most owners don't, and it's an easy thing to get wrong in either direction.

There's a "safe harbor" that keeps you from owing a penalty - for most owners, paying in 110% of last year's tax across the four quarters. But safe from the penalty isn't the same as paid the right amount. Have a big year and you'll still owe in April. Have a down year and you've handed the IRS extra cash that could have been working in your practice.

The good news: right now, with three quarters of real numbers, is the best time all year to check and true up your final payment. This week's post walks through exactly how.

A hard truth about break-even that most practice owners never hear.

The textbook version, revenue equals expenses, is close to useless as a goal. You can hit it and still have paid yourself nothing, set aside nothing for taxes, and banked zero profit. You didn't lose money, but you didn't win either.

Your real break-even is higher. It's the monthly collections that cover your operating costs, your taxes set aside, a fair paycheck for you, your loan payments, and a profit slice of at least 5% on top. Run below it and you don't feel it right away, but you're failing slowly, because you can't fund the practice and pay yourself properly at the same time.

This week's post shows you how to build your real number, including the debt detail almost everyone misses: https://prowisefinancial.com/post/the-break-even-number-most-accountants-get-wrong

A quick reframe that saves practice owners a lot of stress every April.
When a tax bill blindsides you, it's easy to blame the bill. Too high. Unfair. But the tax was going to be whatever it was going to be. What hurt was finding out too late to change it.

By the time your return is done in the spring, the year is closed. Your entity, your retirement contributions, how you paid your family, when you bought equipment, all of those levers are already behind you. Catch your number back in the summer, and they're still live. That's the whole game.

A shocking tax bill isn't a tax problem. It's a timing problem. This week's post shows you how to fix the timing: https://prowisefinancial.com/post/how-to-avoid-a-shocking-tax-bill-before-its-too-late
