The first thing I review when I work with a new client is their chart of accounts.

Itโ€™s broken almost every time. Not catastrophically broken.

Just broken enough that nobody can clearly see profitability.

Too many accounts. Too many vague accounts.

Accounts that duplicate other accounts.

Accounts for things that happened once in prior years and never again.

Hereโ€™s the problem. You run a P&L, and it has 150 line items.

You canโ€™t identify patterns. You donโ€™t know which cost categories are actually the problem.

You canโ€™t compare year-to-year because the accounts changed. The fix is a chart-of-accounts audit.

Delete unused accounts. Consolidate duplicates. Rename vague accounts.

Organize it so you can actually understand what your financial statements are telling you.

Takes a few hours. Changes everything.

Medical practices have metrics. They donโ€™t have KPIs.

There's a difference. A metric is just a number.

A KPI is a number that tells you something important about your business.

Most practices track patient volume and revenue.

Thatโ€™s not enough. Here are the seven that actually matter.

๐—–๐—ผ๐—น๐—น๐—ฒ๐—ฐ๐˜๐—ถ๐—ผ๐—ป๐˜€ ๐—ฟ๐—ฎ๐˜๐—ฒ. What percentage of charges do you actually collect? Target: 92%+. If youโ€™re at 75%, youโ€™re leaving money on the table.

๐——๐—ฎ๐˜†๐˜€ ๐—ถ๐—ป ๐—”/๐—ฅ. How long do patient balances sit before you collect? Target: Under 30 days. Over 60 and you have a problem.

๐—–๐—ผ๐˜€๐˜ ๐—ฝ๐—ฒ๐—ฟ ๐˜ƒ๐—ถ๐˜€๐—ถ๐˜. Total practice costs divided by total visits. Tells you if youโ€™re operating efficiently or not.

๐—ฃ๐—ฟ๐—ผ๐˜ƒ๐—ถ๐—ฑ๐—ฒ๐—ฟ ๐—ฝ๐—ฟ๐—ผ๐—ฑ๐˜‚๐—ฐ๐˜๐—ถ๐˜ƒ๐—ถ๐˜๐˜†. Revenue per provider per hour. Tells you if your providers are actually producing at the level you need.

๐——๐—ฒ๐—ป๐—ถ๐—ฎ๐—น๐˜€ ๐—ฟ๐—ฎ๐˜๐—ฒ. What percentage of claims get denied? Target: Under 5%. Over that and something is broken in your billing or coding.

๐—ฅ๐—ฒ๐—ณ๐—ฒ๐—ฟ๐—ฟ๐—ฎ๐—น ๐˜€๐—ผ๐˜‚๐—ฟ๐—ฐ๐—ฒ๐˜€. Which referral sources send profitable patients? Which ones send problem cases that require more time? Know the difference.

The Job Isnโ€™t Losing Money. You Just Donโ€™t Know It Yet.

One thing Iโ€™ve noticed about construction is that most companies donโ€™t find out whether a job was profitable until itโ€™s already finished.

By then, thereโ€™s nothing left to fix. The labor has already been paid.

The materials have already been ordered. The invoices have already gone out.

Iโ€™ve seen jobs that looked profitable when they started end up producing far less than expected because costs slowly crept up throughout the project.

Nobody noticed until the final numbers came in.

Thatโ€™s where job costing becomes one of the most valuable tools a construction business can have.

It isnโ€™t just about tracking costs.

Itโ€™s about understanding whatโ€™s happening while the job is still in progress.

Are labor costs running higher than expected? Are material costs eating into your margin?

Is the project staying on budget?

Those are questions you want answered before the project is complete, not after.

Profitable Doesnโ€™t Always Mean Cash in the Bank. One thing Iโ€™ve learned about construction companies is that being profitable doesnโ€™t always mean youโ€™ll have cash in the bank.

Thatโ€™s because cash moves differently in construction.

You bill based on project progress.

Your subcontractors and suppliers often need to be paid before you collect from the customer.

Throw retainage into the mix, and itโ€™s easy to understand why cash flow becomes one of the biggest challenges in the industry.

Thatโ€™s why Iโ€™m a big believer in a rolling 13-week cash forecast.

List every active project. Map out when you expect customer payments.

Map out when vendors, subcontractors, and payroll are due.

Then update it every week.

I worked with a business that discovered it would face a significant cash shortfall six weeks before it happened.

Because they saw it coming, they had time to adjust payment timing and secure additional financing before it became a crisis.

Thatโ€™s the value of forecasting. It doesnโ€™t predict the future.

It gives you time to prepare for it.

Thirty minutes each week can save months of unnecessary stress.

Your cash flow problem might not be a cash flow problem.

It might be sitting in a report youโ€™ve never looked at:

The Accounts Receivable Aging Report.

Most business owners donโ€™t review it until cash gets tight.
Over the years, Iโ€™ve seen businesses panic as their bank balances shrank, only to discover thousands of dollars sitting in unpaid invoices.

The issue wasnโ€™t revenue. The issue was collections.

Iโ€™ve also seen businesses realize that certain customers consistently pay 60, 90, or even 120 days late. Once they identified the pattern, they adjusted payment terms, improved follow-up procedures, and cash flow improved.

Hereโ€™s what an A/R Aging Report can tell you:

๏ƒผ Which customers consistently pay late
๏ƒผ How much revenue is still waiting to be collected

Your P&L shows profit. Your bank account says otherwise.

This disconnect confuses business owners more than almost anything else I see as a finance leader.

Hereโ€™s whatโ€™s happening:

โ€ข The P&L tracks revenue when itโ€™s earned, not when itโ€™s collected. So that $10K invoice from last month? Itโ€™s on your P&L as revenue, even if the client hasnโ€™t paid yet. Meanwhile, your expenses hit immediately. Payroll, rent, and software subscriptions all cash out the door.

โ€ข What to look at instead:
Your cash flow statement. It shows actual money movement, what came in, what went out, and whatโ€™s left.

Think of it this way: The P&L tells you if your business model works. The cash flow statement tells you if your business survives.

Both matter. But if youโ€™re only watching one, make it cash flow.

โ€ข Pro tip: Run a 13-week cash flow forecast. It takes 30 minutes to set up and will save you from surprises when a big expense or slow payment hits.

Profit is theory. Cash is reality.
