Employer healthcare costs are projected to rise nearly 10% in 2027, the fourth straight year of increases approaching double digits, according to a new Aon report. Average per-employee costs are on track to top $19,000, and employees are absorbing more of it too, with out-of-pocket expenses up over 10% year over year. Some employers are already responding by trimming plan eligibility or shifting more cost to workers. If you offer group health coverage to your team, this is worth factoring into next year's staffing and benefits budget before renewal season arrives.

A lot of practice owners only hear from their CPA at tax time, and that's usually the first sign something's missing. Good tax planning happens throughout the year, while there's still time to act on it, not after the fact.

If you're explaining PPOs, hygiene numbers, or associate structures to your accountant instead of the other way around, that's worth noticing too. One longtime client put it simply: she doesn't think she'd be where she is professionally without a CPA who actually understood the business she was running.

The federal tax conversation tends to focus on rates, but the tax base, what actually gets taxed, matters just as much. A narrow base needs a higher rate to raise the same revenue, and that gap can push people and businesses toward decisions made for tax reasons rather than sound business ones. The Tax Foundation's latest explainer breaks this down using state sales tax as an example: the more exemptions a state carves out, the higher the rate has to climb to make up the difference.

When thinking about taxes, we often consider the tax rate—or what percentage of something is taxed. But there is another factor in considering taxes: the tax base.

Do any of your dental practice employees receive overtime pay? If so, there’s a new 2026 payroll reporting requirement you should know about.

The federal “No Tax on Overtime” deduction took effect in 2025, but beginning with 2026 Forms W-2, employers must separately report qualified overtime compensation. And despite the name, not every employee or every dollar of overtime pay qualifies.

Now is the time to make sure your payroll provider is capturing the right information, not when W-2 season arrives.

We explain what dental practice owners need to know and what to check with your payroll provider now: https://eandassociates.com/new-overtime-tax-deduction/

Learn how the new overtime tax deduction affects dental practices, including what qualifies and the new 2026 Form W-2 reporting requirements.

Still paying off dental school loans? Changes to federal repayment plans could eventually affect both how much you pay each month and how long you remain in repayment.

PAYE is being phased out, IBR remains an option for certain borrowers, and the new Repayment Assistance Plan (RAP) adds another possibility. But for dentists, choosing a repayment plan shouldn't be based solely on finding the lowest monthly payment.

Student debt can affect practice cash flow, taxes, retirement savings, borrowing decisions, and plans to buy or expand a practice. Understanding how the options fit into your larger financial picture is just as important as knowing what your next payment will be.

Learn what’s changing and what dentists should consider before making a move: https://eandassociates.com/still-paying-dental-school-loans/
