Form 4868 provides an automatic 6-month extension of time to file Form 1040 for individual income tax returns. The form must be filed by the original due date, April 15th.

An extension of time to file is not an extension of time to pay. Interest on unpaid tax is calculated from the regular due date, and underpayment penalties may apply.
