CHANGES IN POLICY FOR APPOINTMENTS:
• THERE WILL BE A 25.00 FOR ANY MISSED APPOINTMENT
• IF MORE THAN 15 MINUTES LATE, TAX INFO WILL NEED TO BE DROPPED OFF FOR COMPLETION
• AMENDMENTS WILL BE COMPLETED WITH A FEE
• PAYMENT FOR SERVICES WILL BE COLLECTED BEFORE TAXES ARE EFILED
• DEPOSITS MAY BE REQUIRED FOR DROP-OFFS OR EMAILED RETURNS
THINGS NEEDED TO COMPLETE TAX RETURN:
• EMAIL ADDRESS FOR END-OF-YEAR MAILINGS****
• VALID PHOTO ID, AND SOCIAL SECURITY CARDS FOR EVERYONE ON YOUR TAX RETURN
• BIRTH DATES FOR EVERYONE ON YOUR TAX RETURN
• ALL INCOME: W2’S, 1099’S, K1’S, 1099 MISC, UNEMPLOYMENT INCOME, INTEREST INCOME
• ALL CANCELLED DEBTS: 1099A, 1099 A
• DISABILITY STATEMENTS: 1099SA, SICK PAY
• SALE OF STOCKS AND BONDS REPORTS
• ALL DEDUCTIONS: MORTGAGE STATEMENTS, MEDICAL, CHARITY, PROPERTY TAXES
• STUDENT LOAN INTEREST AND TUITION PAYMENTS: 1098 T, 1098 E
• CHILDCARE PROVIDERS FED ID NUMBER OR SOCIAL, AND AMOUNT PAID AND ADDRESS
• HEALTH CARE COVERAGE PAPERWORK: 1095 A,1098 B, OR 1095C
• VOIDED CHECK OR SAVINGS CARD FOR ROUTING NUMBER AND ACCOUNT NUMBER
DOCUMENTS NEEDED FOR SELF-EMPLOYED:
• FEDERAL ID NUMBER & NAME OF BUSINESS & BUSINESS ADDRESS
• LIST OF INCOME (1099) (BANK STATEMENTS) & LIST OF EXPENSES FOR THE YEAR
• NAME OF CONTRACTORS & SOCIAL’S FOR 1099’S
• QUICK BOOKS REPORTS IF AVAILABLE
• STATE ID NUMBER IF APPLIES
• PERSONAL PROPERTY ID NUMBER IF NEEDED TO FILE FOR THE YEAR
• BUSINESS REPORTS SHOULD BE SENT TO US BEFORE YOUR APPOINTMENT AND IN JANUARY
DOCUMENTS NEEDED FOR HEAD OF HOUSEHOLD & EIC
• BIRTH RECORDS FOR ALL BIOLOGICAL CHILDREN ON RETURN
• LEASE OR MORTGAGE STATEMENT OR UTILITY BILL WITH SAME ADDRESS
• SCHOOL RECORDS, CHILDCARE LETTER, MEDICAL RECORDS WITH SAME ADDRESS
• IF THE RELATIONSHIP IS OTHER THAN SON OR DAUGHTER THEN YOU MUST HAVE COURT PAPERS OR ADOPTION PAPERS OR LETTER FOR THE AGENCY THAT PLACED THE CHILD WITH YOU.
