This week, we are proud to spotlight Lisa Maciejewski-West, CMC, CMOM, CMIS, CPCO-I — a longstanding leader, educator, and advocate in the medical billing world.

With over 40 years of experience, Lisa has been involved in medical billing, management, and consulting since 1982. In 2006, she founded Gold Star Medical Business Services, providing outsourced billing, compliance, and practice management consulting services to physicians and medical practices nationwide.

Lisa’s expertise spans far and wide — she is a certified Medical Coder, Medical Insurance Specialist, Medical Office Manager, and a Certified Professional Compliance Officer, as well as a CPCO Instructor with AAPC. She has taught billing, coding, compliance, and credentialing for medical associations across the country, and is a regular presenter and webinar contributor for numerous organizations.

AMBA members will recognize Lisa as a familiar and valued leader at our national conferences. She and her team led several focus groups in 2022 and 2023, returned as a speaker in 2023 and 2024, and we are honored to have her serve as the Keynote Speaker for the 2025 AMBA National Conference. Lisa also serves on the AMBA National Advisory Board.

Beyond AMBA, in 2023 Lisa was appointed by Governor Greg Abbott to the Board of Directors of the Texas State Office of Risk Management (SORM), where she now serves as Vice-Chair, helping oversee the Workers’ Compensation Fund and statewide insurance programs.

Most importantly, Lisa is the proud mom of 5 children and “Yaya” to 6 grandchildren — a role she loves even more than compliance. 💜

Lisa received a great question about provider enrollment and credentialing recently. The individual's question was: Does a multi-doc clinic need to establish an NPI for the clinic and bill everything under that? Or does each provider in the clinic need an NPI and the clinic bills under each NPI accordingly?

Over the years, we've been asked variations of this question that really boils down to "How are we allowed to legally bill for services rendered in my practice"? During routine compliance audits, we have uncovered practices that are billing out claims for multiple provider employees under one provider's individual NPI. We have found billing going out under a provider's NPI who was fired a year prior to the date of service. We could go on, but let's get back to the answer for this question.

Typically when credentialing a practice/location and the treating providers, the following steps should be taken:

Once a Medical/Healthcare Corporation is Formed:
1. A group NPI is established for the organization/corporation. A Federal tax ID (EIN) is issued for that corporation, by application to the IRS.

2. Once the Group is established with an NPI, EIN, and they have hired their first employee (it could be the medical provider who owns the group, or a provider who has been hired by the group), they can apply or register their group with the payer and link their employee(s) to the group. Medicare calls this a reassignment. Note: Most payers will not register or credential a group unless there are health care providers who can be linked to the group.

3. If the employee or provider-owner is not individually credentialed with the payer, they need to apply for credentialing.

4. Once the group is credentialed and contracted, and the individual providers are linked to the group, healthcare claims are filed with the GROUP TAX ID as the "pay to entity" (CMS1500 form, box 25). The individual provider's NPI is listed as the "rendering provider" (CMS1500 form, box 24J). Depending on the payer, either the company's name and Group NPI or the individual provider's name and Individual NPI are listed as the Billing Provider (CMS1500 form, box 33 and 33a)

5. What is NOT permitted, is to have one provider linked to the group, and then have other providers in the group billing under the linked provider's individual NPI as if that provider was rendering the services. Each provider should be individually credentialed and linked to the group.
