Capital Region CDPHP members: three changes are coming for 2027, all announced by CDPHP itself.

1. CDPHP is merging with Excellus Health Plan, planned for early 2027. CDPHP says its name, local leadership, and local service stay.
2. Express Scripts becomes CDPHP's pharmacy manager in 2027, replacing Capital Rx. CDPHP notes its 2027 Medicare Advantage pharmacy details are still being finalized with CMS, so your plan's own 2027 documents are the source that counts.
3. A new member ID card with a new number is mailing this fall. Keep using your current card through December 31.
