Most high earners approaching retirement don't realize Medicare isn't a flat rate — and the surprise can cost thousands.

If your household income exceeds $218,000 as a married couple, you're subject to IRMAA: an income-related surcharge on top of your standard Medicare Part B and Part D premiums. At the highest income levels, Part B alone can cost nearly $700 per month — per person.

What makes this especially tricky is the two-year lookback rule. Medicare premiums for 2026 are based on your 2024 income. A large Roth conversion, business sale, or capital gain event today can spike your Medicare costs two years from now — often catching people completely off guard.
