Most people don't think about life cover until they need it. By then, it's too late to get it.

A recent study found that income earners in South Africa currently have cover that meets only 39% of what their families would actually need. 💡 That means most households are one unexpected event away from a very different life.

It's not a comfortable thought. But it's worth sitting with for a moment — and then doing something about it. 🛡️

If you don't have life or dread disease cover in place, let's just have a conversation. No pressure, just clarity.

*This Women’s Month, we asked some of our incredible female advisors a simple question: What do you love most about what you do?* 💜

Their answers reminded us that financial planning is about so much more than numbers. It’s about helping people feel more confident about their future, supporting families through important life decisions, and making a real difference in the communities we serve.

We asked them this question while spending time together at our recent IWCP Community Day, and their passion for what they do came through loud and clear.

Here’s to the women who advise, guide, lead and make an impact every day.

Critical Illness Cover: Why Cancer, Heart Attacks and Strokes Continue to Drive Claims Worldwide

When most people think about life-changing financial risks, they often focus on death, disability, or retirement. Yet one of the most significant threats to long-term financial security is often overlooked: critical illness.

Globally, cancer, heart attacks, and strokes, commonly referred to as the "Big Three", continue to dominate critical illness (CI) insurance claims. In many mature insurance markets, these conditions account for between 50% and 90% of all critical illness claims paid.

As medical technology advances and life expectancy increases, the role of critical illness cover is becoming more important than ever.

The Growing Global Critical Illness Market

The global critical illness insurance market continues to experience significant growth, with projections indicating it will exceed $470 billion in 2026.

This growth is being driven by several factors:

- Increasing rates of non-communicable diseases
- Rising healthcare costs
- Improved medical screening and early detection
- Greater awareness of financial risks associated with severe illnesses
- Growing demand for comprehensive financial protection

While medical outcomes continue to improve, the financial impact of surviving a serious illness remains substantial.

Cancer Remains the Leading Cause of Claims

Across virtually every region of the world, cancer remains the single largest driver of critical illness claims, typically accounting for 35% to 55% of all claims.

Cardiovascular diseases, including heart attacks and strokes, remain the second-largest category.

What is particularly concerning is the growing incidence of serious illnesses among younger age groups. Improved screening and earlier diagnoses are resulting in more claims from individuals still in the prime of their earning years.

This trend reinforces an important reality: critical illness is no longer a risk associated only with older generations.

Regional Trends Shaping the Industry

Asia-Pacific: The Fastest Growing Market

The Asia-Pacific region continues to be one of the largest growth engines for critical illness insurance.

In mature markets such as Singapore and Hong Kong, the "Big Three" conditions account for as much as 86% to 93% of all claims. Even in developing markets such as Thailand and Malaysia, a small number of critical illnesses continue to drive the overwhelming majority of payouts.

Another notable trend is the increase in cancer diagnoses among younger adults, particularly between the ages of 15 and 34.

North America: Early Detection Changes the Landscape

North America remains the largest critical illness insurance market globally.

Advanced healthcare systems and sophisticated screening programs are identifying illnesses at much earlier stages than ever before. While this improves treatment outcomes, it also creates higher volumes of early-stage claims.

As a result, insurers and reinsurers continually refine policy definitions and benefit structures to keep pace with medical advancements.

South Africa: A Significant Protection Gap

Closer to home, South Africa faces a concerning challenge.

Recent industry data highlights that a significant proportion of individuals who suffer or die from critical illnesses do not have dedicated critical illness cover in place.

This creates a substantial protection gap, leaving families vulnerable to:

- Loss of income
- Medical expenses not covered by medical schemes
- Lifestyle adjustments and rehabilitation costs
- Long-term financial strain

Demographic trends also reveal interesting differences. Women tend to claim at younger ages, largely due to breast cancer diagnoses, while men typically claim later, with cardiovascular conditions featuring prominently.

The Survival Paradox

One of the most important developments in modern healthcare is what many insurers call the "survival paradox."

Advancements in medicine mean that a diagnosis of cancer, a heart attack, or even a stroke is no longer necessarily fatal.

This is undoubtedly positive news.

However, survival often introduces a different challenge: ongoing treatment, recovery costs, loss of income, and the possibility of future related illnesses.

As more people survive their first critical illness, insurers are seeing increasing numbers of second and even third claims on multi-claim policies.

In other words, people are living longer—but often with greater financial exposure.

How Critical Illness Cover Is Evolving

To address changing healthcare realities, insurers are redesigning traditional critical illness products.

Tiered and Stage-Based Benefits

Historically, critical illness cover often paid a single lump sum once a qualifying condition was diagnosed.

Today's products are increasingly structured around illness severity and clinical staging.

For example:

- Early-stage cancers may trigger a partial benefit.
- More advanced conditions may trigger a larger payout.
- Additional claims may be available for future unrelated illnesses.

This approach provides greater flexibility and aligns benefits more closely with actual medical outcomes.

The Rise of Predictive Analytics

Insurers are also increasingly using artificial intelligence and predictive modelling to identify trends in health risks.

By analysing large volumes of historical claims and health data, insurers can better understand disease patterns and potentially develop more proactive wellness and prevention strategies.

The future of critical illness insurance may be as much about prevention as it is about protection.

The Financial Planning Conversation We Cannot Ignore

The data tells a clear story.

Cancer, heart attacks, and strokes continue to be the leading causes of critical illness claims across the world. At the same time, diagnoses are occurring earlier, survival rates are improving, and healthcare costs continue to rise.

For financial advisers, this presents an important responsibility.

Critical illness cover should not be viewed as a luxury product or an optional add-on. It forms a vital component of a comprehensive financial plan, helping clients protect their income, preserve their wealth, and maintain financial stability when facing some of life's most challenging health events.

The question is no longer whether critical illness can happen.

The question is whether we have planned appropriately for when it does.
